Tips for Completing the MVAQN Quilt
Registration Form
1. This form should accompany any quilt
that you turn in for an exhibit.
2. To electronically fill in this form,
it is best to delete the underlines.
One way to make this easier is to save a blank form, and then fill in
that form each time you register a new quilt. Save your filled-in form using the SAVE
AS function. In doing this you will
have a completed registration form for each of your quilts for future use.
3. If necessary, you can print the form
and fill it in with pen or pencil.
